- Certificate Form 3231 is the one immunization certificate needed and serves children of all ages.

» NEW Immunization Requirements «
The state of Georgia updated the immunization requirements based on the current Recommended Childhood Immunization Schedule, developed and approved by the Advisory Committee on Immunization Practices, the American Academy of Pediatrics and the American Academy of Family Physicians.
All Morningside student immunization as well as dental, hearing, and vision forms have been reviewed for compliance. Please read the requirement descriptions below and check with your child's pediatrician to update his or her records as needed. Physicians' offices are aware of the rulings and will provide and complete the correct forms as needed.
Varicella vaccine ( chicken pox ) is required for the 2001-2002 school year.
Hepatitis B vaccine was previously required only for both day care attendees and students entering school born on or after 1/1/92. This requirement has been extended to include all new students enrolling in school at any age.
2 - DOSES of a measles containing vaccine ( Mumps, Measles, and Rubella) are now required for all students entering a Georgia school for the first time as opposed to just those students who will be entering the sixth grade. This sixth grade requirement remains the same.
Polis - at least 3 doses with the last dose of polio vaccine being given after the 4th birthday.
